Joe Manganiello is opening up about a devastating health battle that left him fearing for his life and deeply affected his seven-year marriage to actress Sofía Vergara. In his new memoir, Bloodlines, the Magic Mike star describes years of severe pain, unexplained symptoms, major surgery and emotional struggles that unfolded after the couple married in 2015. His story is not just about a relationship ending, but about how a prolonged medical crisis changed his body, his outlook and his plans for the future.

Manganiello married Vergara in November 2015, but not long afterward, he began experiencing health problems that gradually became more severe. He has described suffering from debilitating abdominal pain, fevers, rashes, headaches, fatigue and bleeding ulcers in his digestive tract. As the symptoms continued, doctors struggled to identify the underlying cause, leaving him facing a frightening period of uncertainty while trying to maintain his career and personal life.

Doctors Struggled to Explain His Mysterious Illness

Manganiello says he underwent repeated medical examinations, including numerous colonoscopies, as doctors investigated the damage to his digestive system. Treatments associated with conditions such as Crohn’s disease and ulcerative colitis did not resolve his problems, and he says he never received a definitive diagnosis that fully explained what was happening. The actor has described the experience as exhausting and frightening, particularly as his symptoms continued despite attempts to treat them.

Emergency Surgery Changed His Life

In 2016, Manganiello’s condition became so serious that he underwent emergency surgery that involved removing part of his intestines. He has said he lost approximately 60 pounds and needed an ostomy bag afterward because his digestive system could no longer function normally. The operation marked a dramatic turning point, but it did not immediately end the physical and emotional consequences of his illness.

The Illness Took a Serious Emotional Toll

The physical suffering also affected Manganiello’s mental health, as he spent years worrying that his condition could kill him. He has described reaching a point where he experienced suicidal thoughts and gave his gun away because he was concerned about his safety. During this difficult period, he adopted a tiny Chihuahua named Bubbles, saying that caring for the dog helped redirect his attention toward another living being who depended on him.

Manganiello and Vergara married in November 2015 and announced their separation in July 2023 after approximately seven years of marriage. In Bloodlines, Manganiello describes how the prolonged illness affected their relationship, saying their marriage eventually became emotionally distant during the years he was struggling to survive. His account reflects his own experience of the relationship, and the couple’s separation involved deeply personal circumstances that have been described differently by the two stars.

The Couple Also Faced Difficult Decisions About Having Children

The question of having children also became part of the public discussion surrounding their split. Vergara previously said that differences over parenthood contributed to the end of the marriage, while Manganiello has explained that his serious health problems changed his own outlook on becoming a father. He has said he worried that his condition could take his life and leave Vergara raising a child without him, illustrating how the illness affected decisions far beyond his immediate medical treatment.

Manganiello says he has since undergone reconstructive surgery on his digestive tract and is now symptom-free and off medication. He has described his recovery as feeling like being reborn after years of believing he might not survive another week. Although he has not publicly identified a definitive diagnosis that fully explains his illness, he says he is finally able to look toward the future with less fear.

His Memoir Is About Survival, Not Just Divorce

Manganiello’s memoir, Bloodlines, is scheduled for release on Oct. 13, 2026, and recounts his career, health crisis, personal struggles and path toward recovery. He has emphasized that the book is intended to reveal more of his life and tell a story about survival rather than simply assign blame for the end of his marriage. By sharing the experiences he kept private for years, the actor hopes readers will understand how profoundly the illness changed his life.

A Difficult Chapter Has Given Way to a New Beginning

Manganiello’s account reveals how a serious, poorly understood illness can affect nearly every part of a person’s life, from physical independence and emotional well-being to relationships and future plans. His seven-year struggle included emergency surgery, substantial weight loss, persistent uncertainty and the breakdown of a marriage that began during a happier period of his life. Now reporting that he has recovered, the actor is sharing the experience through Bloodlines as he begins a new chapter with his health, personal life and future in a very different place.
