Between manicures and clipping sessions, it’s easy to treat nails as just another surface to maintain. But dermatologists say the color, texture, and shape of a nail can flag conditions ranging from a simple iron deficiency to liver or heart disease long before other symptoms show up. Here’s what seven common nail changes can actually mean, and when a change is worth mentioning to a doctor.

Those Vertical Ridges Are Probably Just a Sign of Aging

Up to 85% of women over 50 develop fine vertical ridges running from cuticle to tip, and dermatologist Melanie Palm says the cause is usually simple. As skin cells age, the nail plate doesn’t regenerate as evenly as it once did, producing lines comparable to fine lines on the face. A ridge-filling base coat and gentle buffing can smooth them out. Horizontal ridges are a separate story worth a doctor’s attention.

Brittle, Splitting Nails Could Point to a Thyroid or Hormone Issue

Nails that peel or break more easily than they used to often trace back to age-related drops in cholesterol sulfate, the compound that keeps the nail plate flexible. Dermatologist Dana Stern notes that thyroid disease, Raynaud’s disease, and protein deficiency can also surface first in the nails. Switching to a glass file and a gentler, conditioning polish remover can help limit further damage while any underlying cause gets ruled out.

A Spoon-Shaped Nail Can Signal Iron Deficiency Anemia

When nails curve inward instead of outward, forming a slight dip rather than a dome, the condition is called koilonychia, or spoon nails. According to Mayo Clinic, spoon nails are often a symptom of iron deficiency anemia, though a liver condition called hemochromatosis can also be the culprit. Fatigue, pale skin, and shortness of breath are the accompanying clues doctors look for alongside the nail shape itself.

White Nails With a Pink Band Are a Recognized Liver Warning Sign

Nails that turn mostly white except for a narrow pink or red strip near the tip are known as Terry’s nails, and the pattern is tied closely to liver disease. Research cited by the Kingsley Clinic found the pattern present in roughly 80% of patients with severe liver cirrhosis. It can also show up with congestive heart failure, kidney failure, or diabetes, which is why doctors treat it as worth investigating rather than ignoring.

Redness Around the Nail Can Track Back to Blood Sugar

Small red streaks or a flush of redness around the cuticle, known clinically as periungual telangiectasias, show up in roughly half of people living with diabetes, according to a review by physician Rowan Hillson. The same vascular changes that cause the redness can also leave nails brittle and more prone to fungal infections that discolor them white, yellow, or green over time. A simple blood test can confirm whether blood sugar is the cause.

Dark Vertical Lines or Curved Nails May Point to the Heart

Unexplained red or purple streaks running under the nail, or nails that curve noticeably over the fingertip in a pattern called clubbing, can both be tied to heart disease. Extreme fatigue, swollen ankles, and pain in the chest, stomach, or jaw tend to show up alongside these nail changes when a cardiac issue is involved. Any of these signs together are generally treated as reason enough to see a doctor promptly.

Pitting and Nail Separation Often Show Up Before Joint Pain Does

Tiny dents across the nail surface, nails lifting away from the bed, or reddish discoloration underneath can all precede the joint pain of psoriatic arthritis. Up to 80% of people with the condition experience nail changes first, since the same inflammation that targets joints and skin can also reach the nail matrix. Dermatologists generally advise against gel or acrylic manicures for anyone already managing active nail psoriasis.

Small Daily Habits Can Head Off Several of These Nail Changes

Circulation-boosting cardio, daily moisturizing with jojoba oil or a thick cream, and switching to a glass nail file over a cardboard emery board all support healthier nail growth over time. Skipping harsh acetone removers, wearing gloves while cleaning, and prioritizing iron and protein-rich foods over biotin supplements round out the list. Giving nails an occasional break from polish altogether doesn’t hurt either.

When a Nail Change Is Worth an Actual Doctor’s Visit

Most nail changes are harmless and tied to aging or minor habits, but dermatologists draw a clear line on when to get checked. A change affecting only one nail, one that appears suddenly, or one accompanied by pain, swelling, or a new dark streak all warrant a same-week appointment. Otherwise, a routine mention at your next physical is generally enough to keep any underlying cause from going unnoticed.
